Performance Metrics: By the calendar day of each month established in the Task Order, the Contractor shall submit an invoice to the Contract Manager inbox at fdem.recoverycm@em.myflorida.com, or to the DEMES Platform, for all deliverables satisfactorily completed during the previous month. “Satisfactorily completed” shall be determined at the sole discretion of the Division’s Contract Manager. Compensation will be based on contracted hours worked on deliverables satisfactorily completed during the invoiced monthly service period. The method of invoice submittal will be instructed by the Contract Management Supervisor. Payment is limited to activities that are reasonable and eligible under FEMA policy.

Financial Consequences: The Contractor shall provide all reports in the timeframes and format required by the agreement. If the Contractor fails to submit a required report in the manner or timeframe specified in this Agreement, the Agency shall impose a financial consequence, reducing the invoice amount equal to 1 % (for each infraction) of the total invoiced amount for that month the report was due. The Contractor shall request approval from FDEM to make changes in key staff and must fill vacant contractually required positions within thirty (30) calendar days. The Agency shall impose a financial consequence of one thousand dollars ($1,000) per business day for each day that a change in key staff is made but not approved by FDEM, or failure to fill key staff positions.

Small Project Milestone #1 (Deliverable S1) - upon project advancement into Grants Portal “Pending Scope and Cost Development”, all staff hours worked on the activities required for meeting this milestone will be invoiced. 2. Small Project Milestone #2 (Deliverable S2) – upon submission of a payable to Finance (Step 6) in the FloridaPA workflow, all staff hours worked on the activities required for meeting this milestone will be invoiced. For Enhanced Small projects, activities associated with Quarterly Reports may also be invoiced. 3. Small Project Milestone #3 (Deliverable S3) – upon project work completion and acceptance of Attachment D (1A), Small Project Completion Summary Form, all staff hours worked on the activities required for each project for meeting this milestone will be invoiced. No billable hours may accrue beyond 60 days from Applicant’s final request for small project closeout is allowed without express written authorization of the Contract Manager. The Contractor shall complete the Small Project Completion Summary Form only after receiving written confirmation (email is acceptable) from the Applicant that the identified project(s) are complete and that all applicable Environmental and Historic Preservation (EHP) and Insurance Obtain and Maintain (O&M) requirements have been satisfied.

Large Project Milestone #4 (Deliverable L4) – Upon submission and Agency acceptance of the Final Inspection Report (FIR) and advancement of the project to “Awaiting FEMA Determination” in the FloridaPA grant management system, all staff hours worked on final inspection, financial reconciliation, and FIR packet submission will be invoiced. Additional time spent notifying the Applicant following FEMA’s processing of the closeout may also be invoiced. 5. Quarterly Report #5 (Deliverable L5) – upon submission and Agency acceptance of the Quarterly Report, all staff hours worked on allowable activities required to complete the report will be invoiced.
