| Performance Metrics: Deliverables shall be reviewed by the DEP Contract Manager to verify satisfactory completion of the services rendered for each of the tasks, in accordance with the activity and deliverable descriptions contained in the scope of services. |
| Financial Consequences: No payment for unsatisfactory deliverables. In the event that a deliverable is deemed unsatisfactory, the Contractor shall re-perform the services needed for submittal of a satisfactory deliverable, at no additional cost to the Department, within 15 days. If a satisfactory deliverable is not submitted within the specified timeframe, the Department may, at its sole discretion, terminate the contract for failure to perform. |

| Major Deliverable: TASK 2, CO 1 - CC&A will provide a monthly report and form package (Include copies of all reports and logs generated, documents received from the Contractor, forms that may be applicable for the work done during the preceding month, meeting minutes and comments and approvals of any reviewed documents. The report will be provided within ten (10) days of a completed month of construction.

| Major Deliverable: Task 1: Pre-Construction Phase - Provide one electronic copyo f the pre-constructions meeting minutes and the EEO data $1,578.39; Construction Phase - Monthly report and form package $22,887.18 for each nine months ($205,984.62); Post Construction Phase - Project Closeout - provide the Final Inspeciton and Project clsoe-Out Package $10,360.30; Geotechnical Services - Sampling and Testing $3,183.64; FedEx and copies not to exceed $300. In plant bridge fabrication inspection $10,070.76. CO1 - Two additional months of CEI. Structural inspection on the steel bridge at the manufacturing site. Cost not to exceed $55,845.12. The Department shall pay the Contractor on a cost reimbursement plus fixed fee basis not to exceed $277,252.07.

| Performance Metrics: 1) The Project Manager will reveiw the meeting minutes and data to ensure they have complete information and the EEO data and were received within ten days of the meeting. 2) Project Manager will review and verify monthly report contains copies of all reports and logs, documents from the Contractor and that it was received within ten days of completed momth of construction. 3) Project Manager will review and verify the Project Close-Out Package (electronic and hardcopy) including the collected forms and data from the contractor relative to the project completion as listed in the description above and that it was received within ten days of Project Final Completion. 4) Project Manager will review and verify that the Geotechnical Report (2 hardcopies), was received within two weeks following the construction final completion. |
