Major Deliverable: Restitution Inquiries: Assist 32 Holocaust Survivors per quarter, with restitution inquiries pertaining to Holocaust era insurance claims, financial claims, property claims, and acquiring assets and proceeds from European Holocaust settlement funds, or any efforts to address the effects of the non-payment of claims or the non-return of confiscated assets.

Non Price Justification:

Performance Metrics: Report of work provided by Contractor and includes at a minumum: 1) Documentation shall be provided in the following format: achieve complete eligibility reviews, 2) Documentation complies with the procedures and standard provided by the Contract Manager, 3) Achieve complete eligibility reviews

Financial Consequences: For non-performance: $78.12 Holdback for Shortfall (per Survivor not assisted.) Contract is payable quarterly. Modify Contract section 5C, the Provider has the opportunity to "earn back" withheld payment when deliverable is accepted by Department. Applicable shortfall may be invoiced at the end of subsequent quarter. Modify Attachment C, section 1, Note** to add that the Provider shall retain overages and invoice them in a subsequent quarter.

Performance Metrics: Report of work provided by Contractor and includes at a minumum: 1) Files contain evidence of follow-up showing progress with applications,2) Document business and technical requirements for approval by the Department,3)Final acceptance of the Deliverable shall be considered to occur when the Deliverable has been approved by the Department.

Major Deliverable: Outreach Plan: Target a minimum of 100 local Holocaust Survivors per quarter to increase awareness of the new and existing Holocaust restitution programs for which Survivors are eligible to submit claims. Outreach activities will be developed and implemented to assist through education to mitigate the effects of the nonpayment of claims or nonreturn of property to Holocaust Survivors.

Deliverable Price: $2,500.00

Non Price Justification:

Performance Metrics: All performance standards must be met: Plan shows it is: targeted to the size audience required, activities in the Plan are timely implemented per Plan dates, outreach up to standard in the U.S., develop assessments to ensure the plan, meets requirements with no decrease in performance or timeliness of Outreach Plan.

Financial Consequences: For non-performance: $25.00 Holdback for Shortfall (per Survivor not assisted.) Contract payable quarterly. Amounts of any shortfall may be earned back at the end of the subsequent quarter. The Provider shall retain overages and invoice them in a subsequent quarter.
