To support the invoice for this Deliverable, the Contractor shall submit the SDF with Addendums; a roster of Contractor Personnel trained in each Activity, a record of trainings conducted/attended, and Proficiency Levels achieved as evidence of completion of Contract Initiation Services. |

The Contractor shall submit a completed Monthly calendar with applicable Evidence of Work demonstrating appropriate performance of each Activity according to the applicable Activity Definition and/or ticket. |
