Performance Metrics: The Contractor will submit a Quarterly Report, including all of the following items at a minimum: a. documentation of needs assessment for each Participant seeking assistance. B. Documentation demonstrating that a complet eligibity review for restitution programs for which the Participant might be eligible has been conducted. c. Documentation will be in either Word or Excel formats and submitted per the procedures and standards provided by the Department's Contract Manager and will include the identifier for the Participant, date of contact, a description of the request, the name of the social worker, and the status of the request.

Financial Consequences: Failure to satisfactorily complete each task for this deliverable and provide acceptable documentation will result in denial of payment.

Major Deliverable: Provide experienced case managers who provide application assistance and follow-up correspondence to eligible Participants in relation to acquiring financial assets from Holocaust era settlements funds. These services may be provided in the Participant's home or at the Contractor's business location.

Deliverable Price: $105.00

Non Price Justification:

Performance Metrics: the Contractor will submit a Quarterly Report, including all of the following items at a minimum: a. documentation demonstrating that a per Participant needs assessment was conducted. B. documentation validating that assistance for obtaining restitution form European settlement funds was provided to eligible Participants. c. Documentation verifying that the services provided must be maintained within client files and the clients files must be made available for monitoring at scheduled site visits. d. Reports from the Contractor will contain all of the required information requested to the Department Contract Manager and will include the identifier for the Participant assisted, the name of the social worker providing the assistance, dates of contact, type of restitution fund, and amount of restitution sought or type(s) of assistance provided. e. The vionce, documented deliverables, and statistical information as requested by the Department Contract Manager will be submitted ot the Department by the 10th calendar day of the month folloiwng the end of each quarter.

Major Deliverable: Provide outreach and education to potential Participants to increase awareness in the targeted communities of new and existing Holocaust restitution programs and assistance programs that Holocaust Victims may be eligible to receive.

Deliverable Price: $20.00

Non Price Justification:

Performance Metrics: Each of these performance standards must be met: a. outreach plan is reviewed by the Department and demonstrates that it targeted the agreed upon number of participants in the Contractor's geographical area and that the outreach/education activities will be provided in a variety of effective educational ways to incrase awareness of restitution programs. b. outreach plan is submitted within 30 calendar days of the effective date of the Contract and each successive State fiscal year. c. At minimum of three (3) activities in the Outreach plan are timely implemented per Plan dates per quarter. d. Quarterly report submitted to the Department lists the dates and types of outreach/education activities that were implemented, describes how the information was distributed, and provides the number of Participants that participated in these activities. e. The contractor will provide redacted mailing lists of outreach/education activities, samples of the information that was distributed, and redacted sign-in sheets from outreach activities.

Financial Consequences: Failure to satisfactorily complete each task for this deliverable and provide acceptable documentation will result in denial of payment.
