| Performance Metrics: Quarterly Report: The provider shall compile the following additional information and submit with the September, December, March and June monthly reports.
a) Description of any difficulties encountered by direct client services providers and/or their clients.
b) Report on achievement on performance measures.
c) The provider shall submit a quarterly financial report stating by line item, all expenditures made as a result of services provided through the funding of this contract. The report will include certification that these expenditures are true, accurate, directly related to this contract by an individual with legal authority to bind the provider.
and Final Report: The provider shall compile the following information for the entire
contract period and submit with the June final quarterly deliverable:
a) Summary of results from customer satisfaction surveys.
b) Final Expenditure Report and inventory. |

| Major Deliverable: Effective 06/30/11, Renewal 2 Amendment 2 Ensure the availability of an operational Call center. Provider must retain the phone number number 1-866-673-HOPE (4673) to be solely used for the Florida Pregnancy Support Services Program.
b) The call center shall be staffed 24 hours a day, 7 days a week, 365 days a year; to accept calls and emails from women facing unplanned pregnancies and other related concerns.
c) Callers shall receive brief consultations, as warranted, and be directly connected to the direct client service providers within their community that are deemed eligible to participate in the program.
d) Provide statistics on the utilization of the line and routing of calls to the provider network.

| Performance Metrics: Certification Standards and Implementation Plan for FPSSP providers shall be developed and submitted to the department no later than December 31 of the current contract year.
a) The draft standards shall be submitted for departmental approval no less than (3) weeks prior to publication. Any changes shall be returned utilizing track changes.
b) The standards shall not be published and/or implemented without written departmental approval.
c) Provide the department with the final approved standards.
d) Develop and submit an implementation plan with the approval of the department for the providers to be certified for the 2012-2013 contract year.
