Major Deliverable: Provider will perform the following tasks: 1. Create an (IHC) in service area within 60 days after contract execution as follows: a. Identify local health groups and determine their inclusivity. Document findings in QPR. b. Complete the CHII outlining the steps to establish an IHC. Submit to the CM within 45 days of contract execution. c. Establish an IHC based on the CHII. Document findings in the QPR. d. Recruit individuals and organizations to participate in IHC in contract term. Document efforts and participants in QPR. 2. Participate in meetings with Department as requested during contract term. Document in QPR. 3. Participate in at least 1 monthly call with Department and NCHPAD staff. Document in QPR. 4. Participate in all webinar trainings scheduled by Department. Document in QPR. 5. Participate in in-person trainings scheduled by Department. Document in QPR. 6. Acquire the "Prevent T2 for All" program from DOH within 45 days of contract execution. 7. Use completed CHII to develop an implementation strategy for adapted program by 8/30/2018. Select strategies to include PWD. Submit implementation strategies to CM for approval. 8. Implement "Prevent T2 for All" as follows: a. Enroll at least 4 participants in the "Prevent T2 for All" by 12/15/2018. Document enrollees in QPR. b. Host courses as specified in curriculum no later than 12/03/2018. Host courses and events during the contract term. Document hosted course and events in the QPR. Include copies of materials. 9. Prepare a QPR that includes at least the information specified in above tasks and submit to CM within 30 days after end of quarter, but no later than submission of invoice. 10. Evaluate the "Prevent T2 for All" at least once annually and document findings in annual report. Submit annual report to CM by April 30 of each year.

Non Price Justification:

Performance Metrics: a. Identified local health coalitions must be documented as specified. b. The CHII must be completed and submitted as specified. c. An Inclusive Health Coalition must be established as specified. d. Recruitment efforts must be documented as specified. e. Meetings with the Department must be participated in as specified. f. The minimum number of phone calls with the Department must be participated in each month as specified. g. All webinar trainings scheduled by the Department must be participated in as specified. h. In-person trainings scheduled by the Department must be participated in as specified. i. The adapted "Prevent T2 for All" program curriculum most be acquired from the Department as specified. j. An implementation strategy for the adapted "Prevent T2 for All" program must be developed as specified. k. The minimum number of participants must be enrolled for the adapted "prevent T2 for All" program as specified. l. Courses for the adapted "Prevent T2 for All" program curriculum must be hosted as specified. m. A Quarterly Progress Report must be submitted each quarter as specified. n. The adapted "Prevent T2 for All" program must be evaluated each year as specified.

Financial Consequences: 1. Failure to perform tasks 1a) and b) will result in a five percent reduction in the quarterly invoice amount. 2. Failure to performs task 1c) will result in a seven percent reduction in the quarterly invoiced amount. 3. Failure to perform tasks 1d), 3) and 4) will result in a six percent reduction in the quarterly invoiced amount. 4. Failure to perform tasks 2), 5) and 6) will result in a five percent reduction in the quarterly invoiced amount. 5. Failure to perform tasks 7) and 8b) will result in a seven percent reduction in the quarterly invoiced amount. 6. Failure to perform tasks 8a) and 10) will result in a three percent reduction in the quarterly invoiced amount. 7. Failure to perform task 9) will result in an eight percent reduction in the quarterly invoiced amount.
