Major Deliverable: 1). Purchase telemedicine equipment for the provision of health care services by Provider’s nursing and emergency medical staff within 180 days from the date of contract execution. 2). Maintain proof of purchase documentation for the telemedicine equipment and submit it to the Contract Manager within 30 days of the date of purchase, but no later than submission of the invoice.

Non Price Justification:

Performance Metrics: The Deliverables must be met at the following minimum levels of performance: 1) Deliverable B.1.b.1): a.) Purchase telemedicine equipment as specified. b.) Maintain and submit proof of purchase documentation for the telemedicine equipment as specified.

Financial Consequences: 1. Financial Consequences: Failure of Provider to complete or submit a deliverable in the time and manner specified will result in a reduction in payment for that deliverable as follows: a. Deliverable B.1.b.1): 1. Failure to purchase telemedicine equipment as specified will result in a ten percent reduction in the invoiced amount. 2. Failure to maintain and submit proof of purchase documentation for the telemedicine equipment as specified will result in a ten percent reduction in the invoiced amount. 3. Delivery Location and Equipment: a. Delivery Location: Telemedicine equipment must be maintained at one of Provider’s locations in Hendry County Florida including Hendry Regional Medical Center at 524 W. Sagamore Ave. Clewiston Florida, 33440; James D. Forbes Family Care Center at 500 W. Sagamore Ave.; Clewiston Florida 33440; and Hendry Regional Convenient Care Center, 450 South Main Street, LaBelle Florida, 33935. b. Equipment: Provider will be responsible for maintaining any equipment purchased under this contract.
