Line Item Description: BLDG 38 Scope of Services: GLE will perform limited asbestos testing of the Client-identified materials from the above referenced facility in Macclenny, Florida. The scope of services includes the collection of up to three bulk samples (one material) of suspect insulation for Polarized Light Microscopy (PLM) analysis for asbestos. It is further our understanding that the scope of work will be limited to Client-identified ductwork within the structure. GLEs scope of services will generally include the following items: 1. Collection of up to three bulk samples (one material) of the Client-identified materials. 2. Laboratory analysis using Polarized Light Microscopy (PLM) performed by a laboratory accredited by the National Voluntary Laboratory Accreditation Program (NVLAP). 3. Letter report summarizing sample results. GLE can provide the services described above according to the following fee schedule, with options presented that are dependent upon the laboratory turnarounds desired. Options Laboratory Analysis TAT Lump Sum Fee Option 1 6-Hour $860.00 This fee includes the laboratory analysis of up to three bulk samples (one material) for PLM analysis, one mobilization, and a letter report. If additional samples are required, they will be collected and analyzed for either $15/sample (3-5 day TAT), $20/sample (48-hour TAT), $25/sample (24-hour), or $50/sample (6-hour) and the cost will be added to the above fee. However, we will not exceed the lump sum fee presented above without your authorization. We request that upon mobilization to the facility, the Client will provide GLE complete access to all areas of the building that are to be tested
