Line Item Description: This is an agreement with Adventist Health System-Sunbelt Inc. dba Adventhealth Sebring (Provider). Scope of Work: Complete and comprehensive sterilization support services for women, for purposes of this family planning Title X contract program, consisting of the provision of an operatory facility (to include nursing, anesthesiology, and other support staff). Provider will provide sterilization support services to no more than 2 clients referred to it by the Florida Department of Health in DeSoto County (Department). 1) The provider agrees to comply with the Comprehensive Sterilization Services Attachment I (PSMAI FY26-27) 2) Patients with Medicaid or other Insurance are not covered by this agreement. 3) The Department is not responsible for unapproved costs incurred for services provided by an outside contracted provider (i.e., Radiology/Pathology reports sent to outside contractors for read or reporting) 4) Cost per service per client: Maximum number of services is 2 patients at $2,857.18 per client not to exceed a total amount of $5,714.36. 5) Contract Period: Agreement shall begin on July 1, 2024, and end on June 30, 2025, or until funds run out whichever comes first. * * * * * * * * * * * The Florida Department of Health in DeSoto County Authorized Users: Elizabeth Patricia Collins, Dodie Whidden, Penny Pringle, Amanda Johnson
