Line Item Description: BPO DDDP for general hospital procedures, exams, illness, strokes, and medical services for treatment of residents of Agency for Persons with Disabilities APD DDDP residents, housed at FSH. The Provider will provide these services at their facility, or Telemed, in a manner consistent with statutes, rules, and guiding principles of the profession. Residents will be served in accordance with normal treatment practices and APD/PATHWAYS/DDDP staff will be available during examinations to assist in management of the resident with developmental disabilities. APD/DDDP/PW are responsible for the Medicare rate of costs incurred. RATE: A fee schedule/cost sheet does not exist for this type of service due to the unforeseen nature of the medical visits, however, following discounted rates will apply. Normal charge for new patient visit is $339.00, discounted rate $230.00 Normal charge for follow up visit is $260.00. Discounted rate will be $110.00. Normal charge for CPT 10005 is $273.00. Discounted rate will be $180.00.
