Line Item Description: The purpose of this purchase orders is to ensure continuity of the provision outpatient substance use disorder treatment and aftercare services program to offenders on community supervision with the Department, within the 14th Judicial Circuit. The services required include intake screening, assessments, individual counseling, treatment plan reviews, aftercare services, aftercare plan reviews, group counseling, referrals for other treatment services, and any other services described herein. The goal of the Department's treatment program is to offer services which motivate and assist offenders in their personal recovery from substance use disorder, while allowing them to maintain residence and employment in the community. Contract Manager: The Contract Manager is Nicholas Ryan Chavers at 850-717-3669. The total allocation for FY26-27 is $45,000
